Health Index
The
School Health Index is a self-assessment tool developed
by Centers for Disease Control and Prevention (CDC), to
assist schools in identifying physical activity,
nutrition, tobacco, school safety and/or asthma-related
strengths and areas for improvement. It is available
for FREE at the CDC website and can be
downloaded, completed online or ordered through
mail.
The
School Health Index Incentive Program, made available
through a $1 million grant from the Department of
Health, made funds
available to NYS schools that completed the School Health
Index (SHI) and developed a School Health Improvement Plan
based on the results. Minigrant funds (up to $7,000)
were awarded to implement strategies identified in the School Health
Improvement Plan that led to improved nutrition and increased physical
activity through policy and environmental changes.

list of schools that were awarded base grants (up to
$1,000) and mini grants (up to an additional $6,000) in
February 2006 can be found at www.nyshealthyschools.org.
At this time, there are no plans for similar
opportunities for mini grants. Schools are
encouraged to contact their local legislators if
interested in pursuing additional dollars for similar
mini grant opportunities.
